Thank you for your interest in playing with the Benton Bucks Homeschool Basketball team. Each player must provide his Head Coach and the Business Admistrator with the following:

 

 

Head Coach

 

* Your name, age and birthdate

* Your current contact information, including an email address.

* Registration fee payment.

* Confirmation of application for insurance.

* Signed liability waiver/medical release form.

 

 

Insurance

 

All players and coaches are required to purchase an individual membership with the team insurance company, USSFA. Players will not be permitted to play in games without a membership. The cost of this membership is usually about $15.00 for the season, and it provides team liability coverage as well as supplemental (after your primary insurance is exhausted) medical/dental coverage with a $500.00 deductible for all Bucks program activities.

 

Each player is responsible to complete and mail the USSFA application form, with payment, for themselves. Application forms can be obtained from coaches. USSFA will mail you back a membership card.

 

AS SOON AS YOU PUT THIS IN THE MAIL TO USSFA, SEND A MESSAGE TO THE BUSINESS ADMINISTRATOR, STATING WHICH PLAYERS' APPLICATIONS HAVE BEEN SENT.

 

 

Liability Waiver/Medical Release

 

This form also may be obtained from your Head Coach, and it must be returned, signed by a parent, before your player is eligible to play in games. This form does not need to be renewed every season, so if you have already filled one out for your player in past seasons, you do not have to do it again.

 

This form releases Bucks coaches and owners of the facilities we are using from liability in the event of an accident. It also gives the coach or parental supervisor the authority to make emergency medical decisions for your child if you are unavailable. In addition, it gives parents an opportunity to inform us of any health conditions that the player has.
